BILLING AUTHORIZATION
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility:
(i) any credit card or bank account information I supply is true and complete
(ii) charges incurred by me will be honored by my credit card company or financial institution, and
(iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (once online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account on a recurring basis.
I understand that all payments will be collected one month in advance on the first of the month. I have read and understand Dance Dayz Studios’ drop procedures (refer to rules, terms, and conditions) and acknowledge that I am responsible for payment whether or not my child attends classes until I submit my drop request online via the customer portal. Dance Dayz Studio does not provide refunds, with no exceptions.
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
ENROLLMENT AND TUITION
Classes are provided as a continuous enrollment, meaning you can join at any time throughout the season. All tuition is billed one month in advance. ***At sign up you will be charged an annual fee, a prorated amount of the current month, as well as next month’s tuition. In other words, there is a two-month commitment to enroll.*** From there, tuition is automatically billed on the 1st of each month, one month in advance. Electronic billing information is required to be enrolled in class(es) or camp(s). Once you have enrolled with a tuition fee, you are locked into that price as song as your account remains active with us! Meaning, prices may go up for new customers, but you will only be charged your original class fee! If there were going to be a change, you will be notified with at least 60 days or two months notice.
ANNUAL FEE
All actively enrolled students will be charged an annual (anniversary) fee at the time of the initial enrollment. The annual fee is good for one year from your enrollment regardless of whether or not your child is enrolled in a class. The annual fee will automatically be renewed every year (on the 1st of the month of the original enrollment date) as long as your child is actively enrolled. If an enrollment has a future drop request, the annual fee will still be charged accordingly. For example, if your annual fee is due in March and you have a drop date submitted for the end of March, the annual fee will be due. However, if your annual fee is due in April, and you drop at the end of March, your annual fee will not be charged.
Annual fees are not required to enroll in camps or our 6 week summer session.
BILLING AND PAYMENT
All tuition is charged on the first of each month and payments are collected with the electronic billing information provided on the account. The primary authorized parent or guardian is responsible for ensuring payment information is accurate and up-to date. Dance Dayz does not charge late fees, however Dance Dayz reserves the right to drop enrollments for any declined payments and outstanding balances. We operate on a perpetual enrollment basis; meaning, provided you pay your tuition on time, your child automatically remains registered for his/her class for the entire season (through June). By providing your debit card, credit card, or bank account information and opting in to recurring payments, you certify that you have the authority to authorize debits or charges to the payment information identified here. In addition, you authorize Dance Dayz to electronically debit or charge the payment information on a recurring basis in the amount due on or after the due date for your payment.
Your authorization will remain in effect until you notify us that you wish to revoke this authorization by appropriately following our drop procedures (see below). You may opt out of recurring billing by accessing your account online at any time, unless actively enrolled in classes or camps.
DROP PROCEDURE
DDS has a ONE MONTH IN ADVANCE CANCELLATION POLICY. Authorized parent or guardian must submit all drop requests online through the customer portal prior to the 1st of the month (meaning the END of the current month. Any drops submitted on, or after, the 1st of the month will not be processed for that billing cycle nor will it be retroactively approved or refunded). For example, a drop request for March must be received prior to February 1st. Verbal drop requests or emailed written notices will not be honored. Only drop requests submitted online through your Customer Portal (client portal message) will be processed.
You are responsible for payment for your student's classes/camps whether or not your student attends class/camp and until the time you submit a drop request online through the customer portal. If a student stops coming to class without submission of a drop request, then that student's account will continue to be charged. Dance Dayz Studio does not offer refunds.
MAKEUP POLICY
DDS offers makeups with a 30 day expiration. You are prorated tuition the first month only that you enroll. NO Refunds will be available for any programs including, but not limited to, classes, camps, and other events because of our student to teacher ratio. DDS never charges any more or any less for classes. This means that sometimes you may receive five classes, and sometimes you may receive three or less. There are no refunds for classes missed due to illnesses, emergencies, or holidays. Prolonged emergencies, illnesses, or relocation may be an exception and will be determined on a case by case basis. All subjected exceptions are up to the discretion of Dance Dayz Studio management.
HOLIDAYS
Dance Dayz Studio is closed to observe the following holidays*:

DDS is open and operates under normal business hours all other days with inclement weather or evacuations being the exceptions. Dates and duration of holidays are subject to change.
WHAT TO WEAR AND WHAT TO BRING
Athletes may wear leotards, dance skirts, dance dresses, or athletic clothing such as shirts, shorts, yoga pants or tank tops. No street shoes are permitted in the studio. Hair is to be up and pulled back. No jewelry, or large hair ornaments that may cause discomfort during activity. Arrive to class with a water bottle. Please leave valuables at home, as DDS will not be responsible for any items that may be lost or stolen. Any necessary personal items (such as shoes and water bottles) are to be left in cubbies and marked with the dancer’s name.
ARRIVAL AND PICKUP
Please arrive at class or camp on time. DDS is not responsible for monitoring the dropping off and picking up of your child. Parents are responsible for making sure that their child enters and leaves the building safely. We are a drop-off facility meaning that parents are welcome to drop their children off and pick them up after class. We have demonstration days every 6 weeks! Those dates are listed below (under
Parents of children who are not able to use the restroom independently are asked to remain closeby during their child's class. Pictures and videos are permitted of your child only.
DANCER BEHAVIOR
All dancers should act in a respectful and attentive manner. If an instructor determines a dancers behavior unacceptable, the dancer will be asked to excuse themselves from the class into the lobby with a staff member for a break. If an instructor determines that a dancer should no longer participate in instruction due to unacceptable behavior, the parents will be notified and Dance Dayz Studio has the right to refuse further instruction.

MEDICAL AGREEMENT
Health and Medical Information:
The Participant(s) acknowledges and agrees to provide accurate and complete health and medical information to the studio. This includes any known medical conditions, allergies, injuries, or other health concerns that may affect participation in dance classes or activities.
Medical Clearance:
The participant agrees to obtain medical clearance from a qualified healthcare provider before participating in dance classes or activities if the participant has any pre-existing medical conditions, injuries, or concerns that may affect their ability to safely engage in dance.
Emergency Medical Treatment:
In the event of a medical emergency, the participant authorizes Dance Dayz Studio to seek emergency medical treatment, including but not limited to calling emergency medical services and administering first aid as deemed necessary by Dance Dayz Studio’s staff.
Participant Responsibilities:
The participant agrees to:

Follow the instructions of Dance Dayz’ instructors and staff during dance classes and activities.

Inform Dance Dayz’ staff of any changes in the participant’s health status or condition that may affect participation.

Use appropriate safety equipment and techniques as instructed by Dance Dayz’ staff.

Not participate in dance classes or activities if the participant is feeling unwell or experiencing any symptoms of illness.
RELEASE OF LIABILITY
Dance and all physical activities carry an inherent risk for injury. In consideration of my (and/or my child's) participation, I release Dance Dayz Studio, and its representatives, successors, and assigns, from all liability arising from accident, injury, illness, or death that may occur as a result of participation. I authorize emergency medical treatment if necessary.
PHOTO RELEASE OF LIABILITY
I grant Dance Dayz Studio the right to use my or my child(s) photograph or image with or without my or my child(s) name, both singly and in conjunction with other persons or objects, and presentations, advertising, publicity, and promotion relating thereto.
COVID-19 RELEASE OF LIABILITY
I voluntarily agree to assume any risks and accept sole responsibility in connection with my child's attendance. We *do not* require masks to be worn by the children or instructors while dancing.
